We are collecting stories and evidence from parents who have been through the CPS court system. Your voice matters, and your experience can help shine a light on how families are treated, what services were provided (or denied), and where the system failed to make “reasonable efforts.” This questionnaire is designed to document:
Case details – basic information about your case and legal representation.
Services & reasonable efforts – what the court ordered versus what was actually provided.
Possible fraud or false claims – if reports were inaccurate or services billed but never delivered.
Impact – how the process affected you and your children emotionally, financially, and physically.
Contact information – only if you are comfortable and willing to be contacted for future legal action.
👉 Please only answer what you are comfortable sharing.
👉 You may attach supporting proof such as letters, emails, reports, court orders, or receipts.
👉 Your responses are confidential and may support future legal actions, including federal lawsuits or whistleblower claims.

Why This Matters Parents across the country have faced wrongful removals, denied services, and court-appointed attorneys who failed to represent them. By collecting this information, we are building a record to expose systemic failures, support legal reform, and hold agencies accountable.
Your courage in sharing your story helps strengthen this fight for justice.
